Benign Prostatic Hyperplasia – Symptoms & Treatments in iran

If you have reached middle age and have a burning sensation in your urine and difficulty urinating, you are likely to suffer  BPH. Benign prostatic hyperplasia (Also called BPH) is a condition in which the prostate gland is enlarged while not cancerous. Benign prostatic hyperplasia is also called benign prostatic hypertrophy or benign prostatic blockage.

The prostate goes through two major growth periods as a men Age. The first occurs early in puberty, when the prostate doubles in size. The second stage of growth begins around age 25 and continues during  the whole life. Benign prostatic hyperplasia often occurs with the second growth stage.

As the prostate enlarges, the gland presses against and pinches the urethra. The bladder wall becomes thicker. Eventually, the bladder may weaken and lose the ability to completely empty, leaving some urine in the bladder. The narrowing of the urethra and urinal retention (the inability to empty the bladder completely) cause many of the problems associated with benign prostatic hyperplasia.

Benign prostatic hyperplasia Treatment:

Depending on the severity of your prostate condition, Age and your Physical condition you have two treatment options:

1. Medication

  • Alpha blockers. These medications relax bladder neck muscles and muscle fibers in the prostate, making urination easier. Alpha blockers — which include alfuzosin (Uroxatral), doxazosin (Cardura), tamsulosin (Flomax) and silodosin (Rapaflo) — usually work quickly in men with relatively small prostates. Side effects might include dizziness and a harmless condition in which semen goes back into the bladder instead of out the tip of the penis (retrograde ejaculation).
  • Alpha reductase inhibitors. These medications shrink your prostate by preventing hormonal changes that cause prostate growth. These medications — which include finasteride (Proscar) and dutasteride (Avodart) — might take up to six months to be effective. Side effects include retrograde ejaculation.
  • Combination drug therapy. Your doctor might recommend taking an alpha blocker and a 5-alpha reductase inhibitor at the same time if either medication alone fail to achieve  optimum result.
  • Tadalafil (Cialis). Studies suggest this medication, which is often used to treat erectile dysfunction, can also treat prostate enlargement.

2. Surgical treatment

• Transurethral resection of the prostate (TURP):

This is the most common treatment for BPH. During this procedure, your urologist will insert a rigid instrument called a resectoscope into the urethra. This is why it is called transurethral. Inserting the scope this way means no cutting into the prostate. They will then use the charged resectoscope to remove the excess tissue that is blocking the urine from leaving the bladder.

You may be put to sleep with general anesthesia. Local anesthesia for the lower part of the body may be used for this procedure. The average in hospital stay for TURP is 1 to 2 days.

• Transurethral incision of the prostate (TUIP):

This is a similar procedure to TURP. Instead of prostate tissue being removed the bladder neck and prostate are cut to relax the bladder opening, allowing urine to flow more freely. TUIP is mostly successful in  men with smaller prostates.

• Simple prostatectomy:

This method is a surgical procedure in which an incision is made through the abdomen or performed laparoscopic. The inner portion of the prostate gland is removed, leaving the external segment intact.

Risks of benign prostatic hyperplasia surgery:

Surgery for benign prostatic hyperplasia is common and generally very safe. However, there are risks with any surgery, such as those related to anesthesia. Risks after procedures can include those related to urine:

  • Urinary tract infections.
  • Frequent urination.
  • Blood in the urine.
  • An inability to hold urine (incontinence).
  • Painful urination or difficulty urinating.

Benign Prostatic Hyperplasia Surgery Recovery:

• Recovery timeline

You’ll go to a recovery room immediately after surgery. There, hospital staff will monitor your heart rate, breathing, oxygen level, and other vital signs. Once you’ve recovered enough, you’ll be taken to your hospital room.

You’ll stay in the hospital for a day or two after your surgery. If you had laser surgery, you might be able to go home the same day. You’ll get medication to manage your pain.

During this immediate post-operative period your doctors will monitor closely for post-TURP syndrome, a rare but serious condition that occurs when too much fluid is absorbed during the TURP procedure. The initial symptoms include dizziness, headache, and a slow heartbeat, and it can progress to shortness of breath, seizures, and even coma.

A catheter placed through your penis into your bladder will remove urine until your prostate heals. The catheter will stay in place for a few days after surgery. If you go home with your catheter, the nurse will show you how to clean around it with soap and water. Cleaning your catheter twice a day will prevent infection.

Your doctor will give you instructions on how to care for yourself once you get home.

• Home care after surgery

Removing extra prostate tissue should help you urinate more easily and less frequently. Still, it may take a few weeks for you to recover fully. Most men who have this surgery are back to their regular activities in six to eight weeks.

While you recover, you may have:

  • an urgent need to urinate
  • trouble controlling urination
  • pain during urination
  • problems getting and keeping an erection
  • small amounts of blood in your urine

These surgery side effects should subside in time. Tell your doctor if you’re still having problems several weeks after surgery.
